Cancellation Policy
Please be aware we have a cancellation notification policy. We require notification three (72 hours) business days (excluding Saturdays, Sundays, and Mondays) ahead of your appointment; any less will incur a $250 fee. Example: If your appointment is Tuesday or Wednesday, the notification must be received by Tuesday and Wednesday of the previous week, respectively. If your appointment is cancelled within 72 (business) hours, you will incur the cancellation fee. There are no exceptions.
No Show Policy
Patients arriving more than 10 minutes late may result in a shortened appointment or cancellation of your appointment if there is not enough time to complete the consultation. If your appointment is rescheduled or canceled due to late arrival, you will be charged a $75 fee.
If you do not call and do not show up to your appointment you will automatically be charged the no show fee of $250. Patients who are not present for their appointment will be considered a No Show and charged $250 on the day of the missed appointment. A continued history of no-shows (3 or more) may result in a dismissal from OPS. In accordance with state guidelines, a patient may be discharged from the practice following three (3) no-shows in a one-year period.
