All surgery patients are required to opt in Klara for secure messaging and updates!

Step 2. Payment

Depending on your chosen procedure(s) and anesthesia, different clearance documents are required.

Local Anesthesia

Visit notes from PCP within the last year*
Lab work from PCP within the last year*
Eye Doctor visit notes within the last year (dilated eye exam)*
Arrange a driver for the day of surgery

IV and Deep Sedation

Visit notes from PCP within the last year*
Lab work from PCP within the last year*
Eye Doctor visit notes within the last year (dilated eye exam)*
A signed anesthesia clearance form
Arrange a driver for the day of surgery
